Subject Selection Forms

Click on your appropriate grade level for the 2021-2022 school year. You will be submitting your subject selection form as a survey link. You must submit the Parent Signature Page document with your survey or your survey will not be accepted. High school students taking AP, Dual Enrollment, or FLVS courses need to read the contracts and sign in the appropriate areas on the Parent Signature Page.

You will meet with your grade level counselor before completing and submitting this form to understand how to complete the process and which courses are recommended (If you are a new student to JMMAST, you do not need to meet with the counselor in advance).
